Sjogren's Syndrome

I just spent the weekend in the hospital for pleural effusion and the Rheumy ran an Anti-DNA panel (dsDNA AutoAb) and it came back really high. Isn’t this the test for Lupus? I currently have RA and Sjogrens. I’m freaking out a little. One of the symptoms for Lupus is pleural effusion.

anti-dsDNA is not a Lupus specific test although many times seen in those with Lupus. There is not one blood test that diagnosis Lupus and actually there are 11 criteria they look at when diagnosing Lupus and having 4 of the 11 puts you in the probable column. Anti-dsDNA is sometimes present with diseases such as chronic hepatitis, primary biliary cirrhosis, and infectious mononucleosis. It may also be seen in those taking drugs such as procainamide and hydralazine.

I have low grade fever of unknown cause, hair loss, Sjögren’s, and now the effusion and positive ds dna test. She is leaning that way.

twanda I am being treated for both. Hopefully you will get answers in time. Note it can take months to years to get a formal diagnosis for Lupus. Fortunately many of the treatments are the same. I hope you get to feeling better soon.

Have they checked you for Epstein-Barr virus?

Answered Nov 9, 2017    reply

Maricela good question. Not sure. I am going for a whole bunch of tests

Answered Nov 9, 2017    reply

Actually, while I was in the hospital they tested me for EVERYTHING. Blood cultures, nasal swabs, you name it. All negative.

twanda Well it's quite possible that Lupus may be added to your list of diagnosis. Depending on how conservative your rheumatologist is will determine if they take a wait and see approach and re-do labs in three months, or he/she goes ahead and diagnosis you with it. Do you know if your C3 and C4 complements were low. If so, this is also another test if low points to lupus. If high it usually points to acute inflammation or infection.

Maricela C3 is fine. C4 is low

My gut is telling me that I will be diagnosed with Lupus.

Answered Nov 9, 2017    reply

twanda I hope not, but if you are people live long and relatively productive lives with Lupus. I've had ups and downs but for the most part no huge complications that were not easily corrected. Best of luck to you.

Maricela Upon seeing the symptoms, this has been a long time coming. I was diagnosed with Sjögren’s in 2012 and RA in 2013.

Answered Nov 9, 2017    reply

twanda The problem is it is sometimes hard to distinguish between them all. I am being treated for sjogren's syndrome, Lupus, and PsA. Of course I also have Raynaud's and hyperPOTS among my many conditions, but three AI diseases and many times hard to tell what is causing what.

Maricela right?? It’s so frustrating!

Answered Nov 9, 2017    reply

twanda That's when it comes down to symptom management. Hopefully you have a good doctor that can help you with this.
